Will County Residents Wanting the COVID-19 Vaccine Can Fill Out This Survey

In continued preparations for vaccine distribution, the Will County Health Department is requesting that residents of Will County fill out this super-short survey.
I filled it out myself, and it only took about 3 minutes.
According to the WCHD:
This survey was designed to assist the Will County Health Department in planning for Covid-19 vaccine distribution. Questions contained in this survey were developed from limited and preliminary guidance from the Center for Disease Control and Prevention (CDC) & Advisory Committee on Immunization Practices (ACIP).
The webpage also has informational links in regarding to vaccine safety, recommendations, information on the Emergency Use Authorizations from the FDA, and adverse affect.
